Health care delivery & management: Provide community-based programs, services, and facilities to seniors, disabled and ill persons in their homes and communities. Collaborative care is provided by a team that includes nursing, therapy, personal care, home health aide, medical social work, and other related services. Services were provided regardless of race, ethnicity, creed, or ability to pay.
