Concordia Physician Practice

Concordia Physician Practice's charitable purpose is to support and carry out the purposes of Concordia Lutheran Health and Human Care (EIN 25-0969458) (CLHHC) and its affiliated 501(c)(3) organizations by providing medical services to residents of CLHHC as well as residents of the community in Cabot, PA regardless of their ability to pay.
